Abstract

This study aims to identify, analyze, and evaluate operational risks at PDAM Tirta Kencana Samarinda. This research uses a quantitative method with a descriptive approach. Data were collected through closed questionnaires distributed to 84 operational employees. The analysis technique used is the Likelihood Impact Matrix method to measure the probability (likelihood) and impact of each identified risk. The results show that most risks fall into the medium risk category, with two main risks classified as high risk, namely pipeline damage and lack of skilled labor These findings indicate that operational risk management at PDAM is not yet optimal, particularly in infrastructure and human resources, thus requiring improved risk control to enhance service quality.
